You also have to provide proof that you have already unsuccessfully attempted weight loss methods under the supervision of a physician and that you may be suffering from other types of illnesses caused by obesity, such as diabetes and high blood pressure. A psychological evaluation proving that you understand the consequences of gastric bypass should also be included. These documents are necessary in order to ascertain that you are undergoing gastric bypass for health reasons and not just for vanity's sake.
